Thank you for considering Nourish RD for your patient's nutrition care. I work closely with referring providers to keep everyone on the same team, here's what you need to make a referral simple.

Please include the patient's diagnosis/reason for referral, contact information, and insurance details if known. The office team reach out to the patient directly to schedule.
Insurance Accepted
I'm in-network with UHC, Medicare, Husky/Medicaid (CT/VA only) Anthem/BCBS, Cigna, and Aetna. Most insurance plans cover medical nutrition therapy at 100%. Credentialed to bill insurance in CT, VA, NY, RI- self-pay is also available for patients outside these states or plans.
VA patients: please refer through LWell.com.

Visit Format
Primarily telehealth, with in-person visits available upon request at our Groton, CT office. This flexibility makes it easy for your patients to stay consistent with care regardless of transportation or scheduling barriers.
Is a Referral Required?
No, patients can self-refer. However, a referral helps ensure coordinated care and may be required by some insurance plans for coverage. If you're unsure how to structure a referral for nutrition services, contact me directly and I'll walk you through it.
